To request a prescription, please complete the form below. We will transfer the pre-ordered prescriptions to your e-card as quickly as possible (e-prescription). You can collect your ordered medication from your pharmacy after receiving a confirmation email. Your e-card is required for collection at the pharmacy. Please note that only existing patients can take advantage of this service.
